Completing Your Field Trip Registration and Fee Waiver

In order to serve your class, we need you to complete both the Education Registration Request Form and the Fee Waiver Application Form if applying for an educational fee waiver.

Your packet should contain the following:

  • Education Registration Request Form (fill out electronically below)
  • Fee Waiver Application Form (will be emailed by education coordinator)
  • Proof that your group is an accredited educational organization (e.g., letter of accreditation or educational tax exemption letter; note that a tax-ID number alone is not sufficient).
  • A lesson plan that includes national, state, or district standards.
  • A statement explaining how your visit to Oregon Caves ties into your lesson plan.

Fee Waivers

  • Fee waiver requests must be submitted at least three weeks before the field trip.
  • No on-the-spot waivers can be granted.
  • A group’s place in the education program is not guaranteed until all paperwork is received and approved. Confirmation will be sent via email when the waiver is granted.

To be eligible for a fee waiver, the group must represent an accredited educational institution visiting to enhance their curriculum. Fee waivers are not issued based solely on distance to the site or economic hardship. Groups such as Scouting, Rotary, Elks, and similar organizations are not eligible for the educational fee waiver.

If a fee waiver is not granted, the following fees apply:

  • Adults (16 and over): $20.00
  • Youth (15 and under): $15.00

Chaperones

Chaperones are required for all educational groups. If your group qualifies for an educational fee waiver, it will cover the costs for a limited number of chaperones:

  • College students: 1 chaperone per 14 students
  • High school students: 2 chaperones per 13 students
  • Elementary and middle school students: 2 chaperones per 13 students

For example, a group of 20 elementary school students is allowed 4 chaperones at no cost. Any additional chaperones will need to pay the standard tour fee. A chaperone can be a teacher, teacher's aide, or parent helper.

Privacy Act Statement

Authority: Public Law 114-289 National Park Service Centennial Act and 54 U.S.C. 100701 Protection, interpretation, and research in System.

Purpose: To administer education programs for education audiences including but not limited to school groups, scouting groups, extracurricular groups, and home school groups.

Routine Uses: To effectively manage requests for education received by the NPS, the Education Reservation Request Form is used to collect basic education reservation information to facilitate operational aspects of scheduling groups for park education programs, including in-park education programs, ranger in classroom programs, and/or online distance learning programs.

Disclosure: Voluntary, however, failure to provide the requested information may impede the ability to grant your education reservation request.

All applicable parts of the form must be completed in order for your request to be considered. You are not required to respond to this or any other Federal agency-sponsored information collection unless it displays a currently valid OMB control number.
